All applicants are encouraged to attend CCTurlock for a minimum of 6 months prior to completing a Ministry Questionnaire. Everyone involved in any aspect of ministry here is required to have an active Ministry Questionnaire on file. The information provided below is confidential and will be reviewed only by those authorized to do so.

I hereby authorize disclosure of this information to appropriate ministry personnel of Calvary Chapel Turlock for the purpose of approval and placement in various ministry positions. If applicable, I also authorize Calvary Chapel Turlock to contact all sources of reference and conduct any necessary background checks to assess suitability to serve in Children's Ministry.
